The Diabetes Nurse Educator role involves both education and clinical management of
people with diabetes. The Diabetes Nurse Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and proactively managing patient's condition.
Perform professional nursing, consultation, and educational services for persons with diabetes, and provides community health education to achieve the company’s goals and objectives of the business.
Educating /counselling of new and known Diabetes patients
Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection
Educate clients on how to use the Accu-chek glucometer for self-monitoring
Liaise with team to organize diabetes events such as outreaches and support group
Planning and coordination of Chronic Care clinic
Visit diabetes clinics and attend diabetes association meetings
Coordinate and implement the company’s vaccination programs such as hepatitis
